The UK (Scottish) company has developed a proprietary data interoperability platform which allows the integration of existing clinical data with new sources of patient information acquired from digital services, medical devices and wearables to support the management of long-term conditions. It provides consent, security, authentication, curation, storage and integration with systems of records. On top of the platform, digital services have been developed to address specific conditions providing; a patient web app to capture Patient Reported Outcome and Experience Measures (PROMS, PREMS), provide messaging and rehabilitation support; a clinical portal to allow interpretation of the data, and improved management and care planning. In addition AI models are applied to the datasets to predict events and provide proactive care planning. The platform has been used for the digital management of diagnosis (Heart Failure) and ongoing monitoring COPD (Chronic Obstructive Pulmonary Disease).

The INCAREHEART PCP project brings together five procurers who want to radically improve integrated care for patients with Chronic Heart Failure (CHF). They are looking for an innovative integrated care solution that overcome fragmented health and care service provision. They will jointly procure Research and Development services to shape an ICT-enabled integrated care solution supporting the implementation of a comprehensive multidisciplinary and cross-organisational care and support model for people living with CHF across a fully integrated patient pathway. The company seeks implementation and technology partners to support the integration of this PCP project.
